About the User Data Collection Forms:
Your Data Collection Form can include a 728x425 pixel branded header, a description of the sweepstakes or prizes, and 10-12 custom form-field questions specific to what you would like to know about users.
It can also contain an opt-in question for sign up to your e-mail newsletter, and terms and conditions of your sweepstakes offer.
After clicking the submit button, users are automatically re-directed to a "Thank You" page, and then re-directed to your website or relevant splash page.
We can then send a personalized (first name in body of email) HTML auto-confirmation E-mail.
